Comments are for anything the team should know: something you’ve noticed in practice, a change in how someone likes to be supported, or a gap you’ve spotted in the plan. Sharing what you see is part of good support.

Before you start. Comments only become available once you’ve signed the plan. If you haven’t yet, see the Read and sign a plan guide first.

Open the feedback panel

With the document open, tap the speech-bubble Feedback icon in the top right. A panel slides in from the side showing any feedback already left on this plan.

Write your comment and submit

Type your observation in the Add Comment box, then tap Submit Feedback. Colleagues supporting this person, and managers, will be able to see it. Keep it plain and specific, that’s what makes it useful.

Comments don’t change the plan. They’re how you flag what you’ve noticed. Any formal change to the plan itself is reviewed and made by a manager, so a comment is the right place for ‘this is what I’m seeing in practice’.

It appears, and it stays

Your comment shows in the panel straight away, with your name and the time. Comments can’t be edited or deleted, so write yours the way you’d want it read back later. That permanence is exactly what makes the record worth trusting.

You’ve found the plan, read it, signed it, and shared what the team needs to know. Your comment is now visible to the colleagues and managers supporting this person, so what you’ve noticed reaches the people who can act on it.
