Upload a support plan for a Person We Support.

A walkthrough in three short phases. Scroll through at your own pace.

Phase one

Find the right person.

Navigate from the dashboard to the Person We Support whose record you need to update.

Open the dashboard and click People

Sign in to Livy. You’ll land on the dashboard, which gives an overview of your organisation at a glance. Open the People area from the left-hand menu, where every Person We Support has their own record.

Search for the Person We Support

Use the search bar to find the person whose record you need. You can search by name, and the list narrows as you type.

Open their profile

When the person appears, click their name to open their record.

Phase two

Get to the right tab.

Each profile has three tabs. Documents live under the third one.

The profile opens on Basic Information, which holds personal details. Two more tabs sit alongside it: Support Worker and Support Document.

The second tab is the support team

The Support Worker tab shows the colleagues who support this person, grouped by access level. You do not need it this time, so carry on to the third tab.

Open the Support Document tab

Click the Support Document tab. This is where every support plan and supporting document for this person is stored.

Phase three

Upload the support plan.

Add the new document, fill in the details, and save.

Click Upload

Click the Upload button in the top right of the Supporting Documents area. A short form will open.

Fill in the document details

Give the document a clear name, for example ‘Support Plan February 2026’. The description is optional. Use the change description to note what is new or different about this version.

Click Select File

Once the details are filled in, click Select File to choose the document from your computer.

Choose your file

Pick the document you want to upload. It must be a PDF. Then open it to attach it to the form.

Click Upload

Check the details are right, then click Upload to attach the PDF to the form.

Last step

Save your changes

Don’t forget this last step. Click Update in the top right. Until you save, the document is not added to the person’s record. Once saved, the support plan is visible to colleagues on shift.
